Documents you need

Keep clear copies ready. You usually need:

  • The child’s hospital birth record or B-Form
  • Both parents’ CNICs
  • Proof of residence in the Union Council area

How to apply, step by step

  2. Submit the child’s record and both parents’ CNICs.
